Medtronic has implemented a centralized on-line request process for customer-based charitable donations and subsidies. This new process will provide a common, efficient experience across all Medtronic businesses.

The Medtronic Office of Ethics & Compliance (OEC) has launched a central website for this process. For request forms and other information about customer-based donations and subsidies, please visit our website at http://www.medtronic.com/donations/request.

The current donation and subsidy request forms for the Medtronic Spinal and Biologics business formerly found on www.medtronicsofamordanek.com are now located on the central website. Please click here to be redirected to the new site http://www.medtronic.com/donations/request.

Effective May 1, 2008, Medtronic, Inc. began posting donation information on its website at www.Medtronic.com/donations. Any donation requested on or after that date will, if funded, be included in Medtronic's Donations Registry; donations made prior to May 1, 2008 and donations committed to before that date, regardless of when they are funded, will not be included in Medtronic's Donations Registry. Information regarding the Medtronic Foundation's donations is already publicly available at www.Medtronic.com/foundation.
